Stage 3 checklist

GOAL: In this final phase, GG provides case management, coaching, and life skills classes as the client actively works toward financial independence. The client has achieved stabilized living, covering most housing costs while GG provides additional support only in emergencies. The focus shifts to education and community-building. They maintain housing independently and transition away from government assistance.

CTI Phase 3: Final Transfer of Support: Ensure the support network can function independently and formally hand off care, ensuring the individual is stable.
